Minister: People often fall between gaps in care


A new system of combined health and social care for vulnerable people is being launched by the government.

Ministers say by 2018 the two services will be "fully joined up", starting in ten pioneer areas at the end of the summer.

The charity Age UK warns the change could be hampered by a lack of funding for social care.

Care Minister Norman Lamb says too often elderly and disabled people fall through the cracks in the current system.
