A report on the website NASA Watch today said the dome was "dropped and is damaged beyond repair. The accident also damaged some tooling."

NASA has not yet announced the extent of damage and is not saying exactly what happened, Henry said, because the space agency is still "working to put together the facts." NASA and prime contractor Boeing have both formed independent assessment teams, Henry said. The effect on the SLS development calendar is also unclear.

There were no injuries, and the incident happened during "normal operations," she said. The dome was not in NASA's vertical assembly tool at the time, and that tool was not damaged.

The Space Launch System will lift off the pad powered by two solid rocket boosters and main engines fed by liquid oxygen and liquid hydrogen. The liquid oxygen tank is the smaller of the two liquid fuel tanks.
