rich 1.0 Tennessee had already watched one execution collapse in 2026 before Christa Pike was strapped to a gurney on September 30. Tony Carruthers’ May execution was stopped after officials could not establish the required backup IV access. Four months later, Pike received two doses of pentobarbital and survived. Pike’s attorneys say the failure inside her chamber was physical and visible. They allege the IV team used at least seven needles, one of them bent at a 90-degree angle, while Pike offered guidance about where they might find a vein. Witnesses heard her breathing and snoring after the drugs were administered. She was later taken to a hospital, where her legal team said she was unconscious and on a ventilator. Carruthers’ case had already put Tennessee’s medical staffing under scrutiny. Court records say Dr. Mark Fowler testified that he had not placed a central line since 2013 and did not hold hospital privileges. Carruthers’ sister has alleged in a formal complaint that he suffered a stroke and partial paralysis after the failed attempt. Those claims have not been established as medical findings in the public record. Reporting also conflicts on whether Fowler personally participated in Pike’s procedure, so that point remains unresolved.
