Burton Hospitals apologises over failing to help prevent Ayeesha-Jayne's death

Dr Magnus Harrison from Burton Hospital's tells Capital they should have made more checks on Ayeeshia-Jane Smith's circumstances and family life when she was brought into A&E twice before she died.

Dr Harrison says not enough questions were asked by staff who should have been more inquisitive.

He says that more thorough checks are being made with a blanket default to be skeptical with patients and inquire more about their environment.
