A damning report's found 456 patients died at a Hampshire hospital due to an "institutionalised practice" of shortening lives "without medical justification".  It's thought a further 200 people - given powerful pain-relief drugs at Gosport War Memorial - may have been affected between the late '80s and the year 2000. The inquiry's called on the Government, Attorney General and police to act.
