CARDIO-TTRansform tested eplontersen (Wainua) in transthyretin amyloid cardiomyopathy and missed its primary endpoint (rate ratio, 0.89), though a pre-specified subgroup not on background TTR stabilizers showed nominal benefit—a reminder that lowering a biomarker does not guarantee clinical efficacy and that background therapy shapes trial results. SINGLE-AF found that direct oral anticoagulation reduced the primary composite endpoint by 69% in patients with atrial fibrillation at intermediate stroke risk, without increasing major bleeding, though absolute event rates were low. POET II showed that a response-tailored, shorter antibiotic course was non-inferior for selected patients with left-sided infective endocarditis, cutting median therapy from 41 to 26 days.
